First-line therapies for bacterial vaginosis
For uncomplicated BV, an Ohio-licensed physician commonly considers oral metronidazole, metronidazole 0.75% vaginal gel, or clindamycin 2% vaginal cream, in line with CDC guidance. Oral metronidazole is typically taken twice daily for seven days. Metronidazole vaginal gel is usually applied once daily for five days, and clindamycin vaginal cream once daily for seven days. Intravaginal options can be a good fit for people who prefer to avoid oral antibiotics.
Common side effects can include a metallic taste, nausea, or stomach upset with oral metronidazole; alcohol should be avoided during and just after a metronidazole course. Clindamycin cream is oil-based and can weaken latex condoms and diaphragms. Your physician chooses the medication and duration based on your symptoms, allergies, pregnancy status, and prior treatment.
What causes BV?
BV develops when the vagina's normal, lactobacillus-dominant balance is disrupted and other bacteria overgrow. Douching, new or multiple sexual partners, and not using condoms are associated with a higher risk, while the exact trigger is often unclear. BV is not passed on like a classic STI, and it is not caused by poor hygiene. Avoiding douching may help reduce risk, but antibiotics are usually needed once symptomatic BV is established.
