What is low testosterone?

Low testosterone, or male hypogonadism, is when the body does not make enough testosterone and a man has symptoms such as low energy, reduced libido, loss of muscle, or low mood. Diagnosis takes both a blood test confirming low levels and matching symptoms, which is why testosterone replacement therapy is prescribed from lab work and a clinician review — never from symptoms alone.

How low testosterone is diagnosed.

A TRT diagnosis is based on blood work plus symptoms, not a questionnaire. Clinical guidelines point to a total testosterone in the range of about 300 ng/dL as a marker for low testosterone, but no single number decides treatment on its own. Testosterone is best measured in the morning, when levels peak, and your clinician reads your result in the context of your symptoms, your full lab panel, and your health history to judge whether TRT is right for you.

Risks, side effects & monitoring.

Testosterone therapy is effective, but it is a real medication with real effects, which is why monitoring is built into every plan. The markers your clinician watches:

  • Red blood cell count (erythrocytosis). Testosterone can thicken the blood by raising hematocrit. Your clinician tracks it on every follow-up panel and lowers or pauses your dose if it climbs too high.
  • Estradiol. Some testosterone converts to estrogen. Levels are checked and managed when symptoms call for it, rather than blocked by default.
  • Fertility. TRT suppresses LH and FSH and can reduce sperm production, so it is generally not appropriate while you are trying to conceive. Read how TRT affects fertility.
  • Prostate health (PSA). PSA is checked at baseline and on follow-up, and treatment is not started when there are unexplained prostate findings.
